The Sheboygan County End of Life Coalition is a collaborative effort of area organizations and their resources to develop and implement strategies to support Advance Health Care Planning in Sheboygan County.

History

The Sheboygan County End of Life Coalition was formed in year 2000 as a collaborative effort to educate the community regarding the physical, emotional and psychosocial care at the end of life to promote a sense of dignity and meaning for individuals and their families.

Coalition Activities

The Coalition provides educational opportunities for the community regarding Advance Directives, end of life options, and decisions through group presentations or one-on-one meetings. Each presentation is tailored to suit the requirements of the target audience, and can range from a comprehensive 6 hour Power Point directed course to a 20-minute overview on the importance of advance directives. What is an Advance Directive?

An Advance Directive is a legal written document that states a person’s choices regarding healthcare. It includes personal wishes based on beliefs and values. This document can give peace of mind to family members when faced with difficult decisions that could otherwise be a spiritual and moral dilemma.

**Coalition members are well versed in the completion of advance directives, also known as the Power of Attorney for Healthcare, and Living Wills and can be contacted for follow-up sessions in order to assist persons in finalizing their individual documents.
